Once those things are loose, things just get worse. A common problem when people rebuild their front 80 series axles is to not get them tight. You can torque the bolts and there still can be a small gap that is hard to see. There is a trick that you tighten the bolts, smack the steering arm with a hammer from the underside and keep torquing the bolts.

Kevin and Ricardo said they never took it off when the reworked the axle a few weeks ago.

Could be it had a small gap and the gap has built up over time while Kevin wheeled it.

Guess we will never know
